Travel prepared

Travelling to a faraway country? Start your preparation on time - preferably 3 months in advance. Check whether you need vaccinations and arrange them through the GGD or another provider. Take the right medication with you and read how to avoid illness while travelling. And perhaps most importantly: leave well-rested and well prepared so you can enjoy carefree travel.

Taking your own medicines on a trip

Here's what to watch out for if you are already taking medication at home:

  • Take enough medicine with you for the whole trip.
  • Take your medicines with you in the pharmacy packaging.
  • Bring a list of the medicines you take. This is called a medicine summary or medicine passport.
  • For some medicines, you need an additional declaration. This is called a medicine declaration.

Medicine review

When travelling abroad, it is best to bring a list of the medicines you are taking (medicine summary). This will help you better and faster when you need to see a doctor or pharmacy.

It says this:

  • the name of the medicine
  • how much you use per day
  • what you use it for
  • whether you have an allergy to medicines

You can get a medicine list printed at your pharmacy. Or make the list yourself. Preferably do this in English or in the language of the country you are travelling to. Keep the medicine list or the note with your ordinary passport or ID card.

Medicine statement

Some medicines are not allowed to be taken abroad. For example, medicines for ADHD, strong painkillers, sleeping pills and sedatives. To take these medicines with you, you need a medicine declaration. On this you fill in which medicines you are taking.

Do you need a statement? Then request it from the CAK or download the form below.

Here's how it works: You can fill in the statement digitally or print it out and fill it in by hand. Hand in the completed declaration to the practice. After a few working days you can collect the declaration, stamped and signed.

Note: costs are not reimbursed by your health insurer. We charge €15.93 for this, payable by pin or cash. Please arrange this at least 4 weeks before departure - as it may take several weeks for your statement to be ready.
