Here's what to watch out for if you are already taking medication at home:
When travelling abroad, it is best to bring a list of the medicines you are taking (medicine summary). This will help you better and faster when you need to see a doctor or pharmacy.

You can get a medicine list printed at your pharmacy. Or make the list yourself. Preferably do this in English or in the language of the country you are travelling to. Keep the medicine list or the note with your ordinary passport or ID card.
Some medicines are not allowed to be taken abroad. For example, medicines for ADHD, strong painkillers, sleeping pills and sedatives. To take these medicines with you, you need a medicine declaration. On this you fill in which medicines you are taking.

Do you need a statement? Then request it from the CAK or download the form below.
Here's how it works: You can fill in the statement digitally or print it out and fill it in by hand. Hand in the completed declaration to the practice. After a few working days you can collect the declaration, stamped and signed.
Note: costs are not reimbursed by your health insurer. We charge €15.93 for this, payable by pin or cash. Please arrange this at least 4 weeks before departure - as it may take several weeks for your statement to be ready.
