We would like to inform you about a change in our invitation system for annual check-ups for patients with cardiovascular diseases. From 1 January 2025, this check-up will take place in the month in which you were born. This makes it clearer, so that everyone knows exactly when the next check-up is scheduled.
From 1 January 2025, you will automatically receive an invitation with a blood test form by post in your birth month. Due to this change, you may be called a little earlier or later than usual. If you do not receive an invitation, or if you think the call comes too soon or too late, feel free to let us know. We will be happy to work with you to find a suitable solution.

We fully understand that you may not be able to attend the appointment. In that case, please contact the practice at least 24 hours before the appointment to cancel the appointment. If you do not cancel in time, we will charge €50.00 for expenses.
