Register with Keizer Karelpark GP practice

New patients are welcome at Keizer Karelpark General Practice. You can easily register via this registration form. You can register with us if you do not yet have a GP in Amstelveen and you live in postcode area 1185.

Practical information about your registration

• On your first visit, we ask you to bring valid identification (passport or ID card) and your health insurance details.
• By registering with us, you give us permission to deregister you from your previous GP. We also request that you inform your current GP yourself that you are switching.
• Please note that completing and submitting the registration form does not automatically mean that you are immediately registered. Once we have received your form, we will contact you as soon as possible to let you know whether registration is possible.
